This is a sample record for an organization. Note that the 'Type' selection is 'Agency/Entity' indicating this record is for an organization.

Also notice how the phone number tags change. Rather than being typical person values like home phone, work phone, cell phone, the labels change to more closely match an organization's form of phone like a primary number, secondary number, and fax.
